Semaglutide

A+ 87.5/100

Evidence Summary

Semaglutide is the highest-evidence longevity compound in the ClinEvident registry. Originally approved for type 2 diabetes (Ozempic) and obesity (Wegovy), it has demonstrated significant benefits across multiple organ systems in large randomized controlled trials.

The FLOW trial (NEJM 2024) showed a 24% reduction in composite kidney outcomes. The SELECT trial showed a 20% reduction in major cardiovascular events in non-diabetic adults with obesity. These are hard clinical endpoints from dedicated Phase 3 RCTs — the gold standard of evidence.

Critical safety note: GLP-1 therapy causes 30-40% lean mass loss alongside fat loss. Resistance training is a MANDATORY combination to prevent sarcopenia. Cold water immersion post-resistance training is directly counterproductive for GLP-1 patients.

Safety Considerations

30-40% of weight loss is lean mass without resistance exercise countermeasure. Resistance training is MANDATORY. GI side effects (nausea, vomiting) common in titration phase. Pancreatitis risk (rare). Contraindicated in personal/family history of medullary thyroid carcinoma or MEN2. Do not combine with cold water immersion post-resistance training.
