GI side effects (diarrhea, nausea) common at initiation. Vitamin B12 depletion with long-term use — monitor levels. Lactic acidosis risk (very rare, primarily in renal impairment). MASTERS trial confirmed blunting of exercise adaptations — timing separation recommended. Contraindicated in eGFR <30.
