Evidence Summary

Metformin has extensive observational evidence suggesting longevity benefits, including the landmark UKPDS study showing reduced all-cause mortality in diabetic patients. A 2024 Nature study showed 4-6 year DNA methylation age rescue in primates.

However, the TAME trial (Targeting Aging with Metformin) — designed to establish aging as a treatable indication — remains incompletely funded as of 2026. Without TAME results, metformin's longevity evidence stays observational.

Critical interaction: The MASTERS RCT confirmed that metformin blunts exercise-induced muscle adaptations via AMPK pathway competition. This is built into our interaction database as a documented antagonistic interaction.

Safety Considerations

GI side effects (diarrhea, nausea) common at initiation. Vitamin B12 depletion with long-term use — monitor levels. Lactic acidosis risk (very rare, primarily in renal impairment). MASTERS trial confirmed blunting of exercise adaptations — timing separation recommended. Contraindicated in eGFR <30.
